CASE MANAGEMENT

BEHIV’s case managers assess a client's HIV-related needs, provide information about available resources and to develop a service plan designed to meet their needs.

Case managers are knowledgeable about the resources available to people living with HIV and AIDS, including
  • medical providers
  • treatment programs
  • entitlement programs (i.e. public aid, Social Security and veterans' benefits)
  • housing
  • mental health services
In addition to partnering with the AIDS Foundation of Chicago to provide Case Management services through the Ryan White CARE Act, BEHIV participates in the State of Illinois, Department of Rehabilitation Services Case Management program, which assists persons disabled due to HIV/AIDS with receiving homemaker services.
